If your child has a medical condition that the school should be aware of please ensure that the school office has all the relevant information so we can help them while they are at school. A Pupil Health Condition Information Form is available from the school office.

​If your child needs to keep any medication in school, please complete a Medication Agreement form. It is your responsibility as a parent to ensure any medication we have for your child is in date.

​If your child needs short-term medicine, such as antibiotics, that cannot be given outside school hours, please bring it to the school office, in the original packaging as provided by the pharmacist or GP which clearly states your child's name, the dosage required and an expiry date. You will also be required to complete a School Medication Agreement Form.
